Images are almost always the biggest contributor to file size. Compression reduces image resolution and re-encodes them more efficiently, which is why photo-heavy PDFs shrink the most, while text-only documents often don't shrink much since there's little to compress.
Embedded fonts, metadata, and any unused objects left over from editing get cleaned up during compression, this is invisible to you but adds up, especially in files that have been edited or re-saved several times.
Scanned PDFs benefit differently, since the whole page is really one large image. If a scanned file also needs to become searchable text, running OCR PDF first and compressing afterward keeps the text layer intact rather than compressing before OCR has a chance to read it clearly.
